Should you want to become a member of the CMS Provider Network, please
forward the following information, as well as the CMS LLC Provider Application (downloadable below) for each Doctor within your practice at your convenience.

  • Copies of your State Chiropractic License
  • Declaration page of Malpractice Insurance
  • Malpractice and Claims History
  • Letter of authorization for release of information
  • CPR Certification Card
  • Chiropractic College Diploma
  • Most recent Curriculum Vitae (CV)
  • Application fee

Upon receipt and validation of the above information, CMS LLC’s Credentialing Committee will review all the applicant documents. The Credentialing Committee’s recommendation will be evaluated by the Executive Board.

Chiropractic Credentialing Criteria

  1. License (includes State and Medicare): All providers participating in the CMS chiropractic panel must be fully licensed. Dual licensing and malpractice history from other states are also examined. A limited license, a history of suspension or probation is not acceptable. The CMS Credentialing Board will review applications. All conditions required by state licensing board must be met which includes current pocket license, CPR certification, and malpractice insurance.
  2. Education: Chiropractors educated in foreign universities must meet additional educational criteria, including national and state board exams.
  3. Continuing Education: All chiropractors licensed in the State of Wisconsin must have 40 hours of continuing education every two (2) years. CMS places the greatest value on continuing education courses that emphasize topics related to the chiropractor’s area of specialization, as opposed to such items as office management and billing procedures. Doctors with less than one (1) year experience are accepted with a probationary rating. Each provider is responsible for maintaining current requirements in other states and with associated examining boards.
  4. Board Certification: Providers will be National Board of Chiropractic Examiners certified or Board eligible (minimal parts 1-3).
  5. Malpractice Record:
    A. The Chiropractor’s malpractice history should involve no lawsuits. Any history of malpractice settlements are investigated on an individual basis.
    B. All mediation of patient complaints is investigated on an ongoing basis.
    C. CMS monitors regulatory action to identify potential/future problems.
    D. Any open civil action is cause for pending a new application or investigation for existing providers.
  6. Chiropractor’s Office:  Each chiropractic office must meet minimum standards, including x-ray capabilities or a contract relationship with a local x-ray facility. No contracts will be offered prior to establishing a fully operational office location.
  7. Application requirements: The CMS Provider Credentialing Application must be complete and acceptable.
  8. Liability Insurance: Proof of active participation at state-mandated levels is required, and a five-year claim history from NCMIC or other carrier. This is obtained from the Provider’s carrier by the Provider.  Release form (57KB pdf) as a certificate holder must be included.
  9. Re-Credentialing: Performed on an annual basis. Failure to respond to re-credentialing requests within allotted time frame may result in contract termination.
  10. Billing Procedures: Provider must follow CMS LLC Billing Procedures (61KB pdf).
